尼拉帕利（Niraparib，在香港的商品名：则乐），是一种高效、选择性的每日一次口服小分子聚（ADP-核糖）PARP1/2抑制剂，用于晚期复发性卵巢癌的维持治疗，可显著延长晚期卵巢癌病人的生存期，减少卵巢癌的复发。

尼拉帕利于2017年3月在美国获得FDA批准上市，同年11月在欧洲获批，用于对含铂化疗完全或部分缓解的复发性上皮卵巢癌、输卵管癌或原发性腹膜癌患者的维持治疗。基于在美国和欧洲的获批，则乐（尼拉帕利）已于2018年10月在香港获批上市。

PARP抑制剂（PARPi）是基于卵巢癌精准治疗的药物，也是卵巢癌治疗史上的一大突破。目前，全球已有三种PARP抑制剂（尼拉帕利，奥拉帕利，芦卡帕利）获批用于卵巢癌的治疗，其中尼拉帕利是首个在美国获批不用检测BRCA的维持治疗的PARP抑制剂。

研究结果表明，在“化疗间歇期”中使用尼拉帕利维持相较于安慰剂，无论病人是否存在BRCA胚系突变都有奇效：

（1）在gBRCA突变阴性患者中，疾病进展风险降低了55%，PFS延长到2倍以上（9.3个月对比3.9个月）；

（2）而在gBRCA突变阳性患者中有更为显著的效果，疾病进展风险降低了73%，PFS延长到近4倍（21个月对比5.5个月）。这提示，BRCA突变是尼拉帕利治疗获益的有利因素，相较于奥拉帕利和芦卡帕利，这种研究更加全面。

（3）在gBRCA突变阴性但HRD阳性的患者中效果也很显著，疾病进展风险降低了70%，PFS延长3倍以上（12.9个月对比3.8个月）。

尽管三种PARP抑制剂（尼拉帕利、奥拉帕利、芦卡帕利）均获美国FDA批准用于复发性卵巢癌的维持治疗，但三种PARP抑制剂还是具有一定区别：

（1）三种PARP抑制剂的药理和药代动力学特性不同。尼拉帕利生物利用度高达73%和更长的人体半衰期（36h），在肿瘤组织中高暴露量而显示出更好的抗肿瘤活性；同时由于尼拉帕利特殊的代谢途径，服用其他药物不需要对尼拉帕利进行药物剂量调整。

（2）患者用药的便利性与依从性不同。尼拉帕利是每日服用一次，临床应用起来非常方便，患者依从性高。而奥拉帕利、芦卡帕利一般需要一天服用2~3次。并且，卵巢癌患者服用尼拉帕利不需要做BRCA基因检测，患者无论有没有BRCA基因突变，都能获得疗效。

（3）患者用药安全性不同。尼拉帕利用于一线化疗后维持治疗的PRIMA研究结果显示，体重小于77 kg的患者若初始剂量为200 mg，每天1次，则其血液学毒性明显减少，且不影响药物疗效。尼拉帕利基于体重和/或血小板计数的个体化给药方案，可降低血液学治疗突发不良事件的发生率。

在2018年，全球范围内有超过30万女性被诊断为卵巢癌。仅在中国，每年就有5万多女性被诊断，2万多人因此而去世。

卵巢癌之所以难治，一个核心原因就是早期没有症状，极难发现和筛查，所以基本一发现就是晚期，疾病已经转移，治疗起来比较困难。这点和“癌中之王”胰腺癌比较像。 以往，卵巢癌维持治疗使用的主要是化疗，效果不够理想，由于缺乏更好的药物，患者只能等待和观察。但尼拉帕利为代表的PARP抑制剂获批后，打破了30年的囧境，明显延长患者的无进展生存期，并且减少复发次数。

适应症:

尼拉帕利是一种聚(ADP-核糖)聚合酶(PARP)抑制剂，适用于有复发性表皮卵巢癌、输卵管癌、原发性腹膜癌的成年患者（需对基于铂化疗有完全或部分缓解）的维持治疗。

用法与用量:

1. 推荐剂量是300m（3粒），口服，每天1次，有或无食物服用。

2. 继续治疗直至疾病进展或不可接受不良反应。

3. 对不良反应，考虑中断治疗，剂量减低或给药终止。  

常见不良反应：

恶心（74%）、血小板减少症（61%）、疲劳乏力（57%）、贫血（50%）、便秘（40%）、呕吐（34%）、腹痛腹胀（33%）、嗜中性白血球减少症（30%）、失眠（27%）、头疼（26%）、食欲减退（25%）、鼻咽炎（23%）、皮疹（21%）、高血压（20%）、呼吸困难（20%）、腹泻（20%）、口腔炎/黏膜炎（20%）等。

NORFLOX

Norflox is an antibiotic in a group of drugs called fluoroquinolones (flor-o-KWIN-o-lones). This medicine fights bacteria in the body.

MEDICINE USES :

Norfloxacin is used for treating bacterial infections. This antibiotic treats only bacterial infections. It will not work for viral infections (such as common cold, flu).

NORFLOX NORFLOXACIN TABLETS  PRECAUTIONS :

    Norflox may cause drowsiness, dizziness, or light-headedness. These effects may be worse if you take it with alcohol or certain medicines.
    Long-term or repeated use of Norflox may cause a second infection. Tell your doctor if signs of a second infection occur. Your medicine may need to be changed to treat this.
    Avoid large amounts of food or drink that have caffeine (eg, chocolate, cocoa, coffee, cola, tea).
    If you experience pain or swelling of a tendon or weakness or loss of use of a joint area, notify your doctor right away. Rest the area and avoid exercise until your doctor gives you instructions.
    Lab tests, including complete blood cell counts and liver and kidney function, may be performed while you use Norflox. These tests may be used to monitor your condition or check for side effects. Be sure to keep all doctor and lab appointments.
    PREGNANCY and BREAST-FEEDING: If you become pregnant, contact your doctor. You will need to discuss the benefits and risks of taking Norflox while you are pregnant. It is not known if this medicine is found in breast milk. Do not breast-feed while taking this medicine.

DRUG INTERACTIONS :

    Antiarrhythmics (eg, amiodarone, disopyramide, dofetilide, quinidine , sotalol), cisapride, diuretics (eg, furosemide, hydrochlorothiazide), macrolide or ketolide antibiotics (eg,  erythromycin, telithromycin), medicines for mental or mood disorders, medicines that may affect your heartbeat, phenothiazines (eg, chlorpromazine), or tricyclic antidepressants (eg, amitriptyline) because the risk of serious side effects, including irregular heartbeat and other heart problems, may be increased. Check with your doctor or pharmacist if you are unsure if any of your medicines may affect your heartbeat
    Nonsteroidal anti-inflammatory drugs (NSAIDs) (eg, ibuprofen) because the risk of side effects, such as seizures, may be increased
    Corticosteroids (eg, prednisone) because the risk of tendon damage may be increased
    Probenecid because it may increase the risk of Norfloxacin’s side effects
    Nitrofurantoin because it may decrease Norfloxacin’s effectiveness

NOVISOF L is a combination of two hepatitis C virus (HCV) antiviral drugs in a single form. Each single tablet contains ledipasvir and sofosbuvir.

MEDICINAL USES :

NOVISOF L is a combination of ledipasvir and sofosbuvir and is used to treat chronic (long-lasting) hepatitis C, a viral infection of the liver. It may sometimes be used with another antiviral medication (ribavirin). These drugs work by reducing the amount of hepatitis C virus in your body, which helps your immune system fight the infection and may help your liver recover.

NOVISOF L COMMON SIDE EFFECTS :

Tiredness, headache, nausea, diarrhea, or difficulty sleeping may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly.

A very serious allergic reaction to this drug is rare. However, get medical help right away if you notice any symptoms of a serious allergic reaction, including: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.

forms of birth control (such as condoms, birth control pills) must be used
whenever at least one sexual partner is using these medicines together, and for
six months after stopping treatment. If you or your partner becomes pregnant,
or if you think you or your partner may be pregnant, tell your doctor right
away.

It is unknown if
this medication passes into breast milk. Consult your doctor before
breast-feeding. Breast-feeding is not recommended if this medication is used in
combination with ribavirin.

NOVISOF L INTERACTIONS :

Some products that may interact with this drug include: amiodarone, a certain other medication used to treat chronic hepatitis C (simeprevir).

This medication can slow down the removal of other medications from your body, which may affect how they work. One example is rosuvastatin, among others.

Other medications can affect the removal of ledipasvir/sofosbuvir from your body, which may affect how this medication works. Examples include rifamycins (such as rifampin, rifabutin), St. John’s wort, tipranavir/ritonavir, drugs used to treat seizures (such as phenobarbital, carbamazepine, phenytoin), among others.

Do not take this medication with other products that contain sofosbuvir.
